        I have basically the same gear and use the LowePro 85AW [ eos3
camera with PB-2].
Also carry 28-135 and 50 macro, 1.4X or 2X TC and small flash [only two of
the latter three or an extension tube instead will fit with the big lenses].
It packs very tight.  I carry from shoulder strap, as I found the belt rides
too high.
        I consider the arrangement a partial success.  With camera attached
to 100-400, have to reverse the hood, but it is fairly convenient.  With
some other lens attached, there is no convenient way to lodge in the bag
with the 100-400.  With 1.4X on 100-400 and camera, it does zip, but just
barely closes.
        After I bought the 85, Kinesis started advertising a long lens bag
that is wide enough at the bottom to accomodate the 100-400 hood unreversed.
Have not tried this, but it might work better than the 85, using the
miniTrekker to hold most of your gear.
