Looks like your lens is faulty. Your best bet would be to sell the lens. Since you have sent it in to Canon 4 times, you can try pressurising them to replace the lens with a new one. I have had a similar problem with my old Canon 100/2.8 (non-USM) macro lens. It died 3 times within 6 months of purchase. Canon refused to replace the lens. I wrote a strong complaint to Canon Japan, and the boss at the local headquarters. They promptly extended my warranty to 2 years. One of the big bosses at the local HQ quickly called me and apologized - they also reassured me that even after the warranty expires, repairs (if any) would be free of charge.
