I have EOS50+380EX and did test three years ago. The
result is the same as describe. What I did is to put
dark subject in the center with different amount of
white wall in the back (typical snap shot of people).
E-TTL does a pre-flash first (then the main flash). It
seems that it measures the overall light reflected and
decided that it is in bright daylight and -1.5 stop is
used on flash power. There you got various degree of
underexposure. 

Nowadays, I just do a flash compensation of +1.5 Stop
that cure some of the problem (while some other
pictures over exposured).
