How about the Manfrotto/Bogen (or any) 1/4" to 3/8" adaptors?
I didn't see the image on the Manfrotto page but I believe
it is the item Bog:3367 (Manf:088LBP) which converts the
3/8 female thread to 1/4 male thread. It is maybe 1/2-3/4" thick
and say 2" wide round metal device.

Of course you can't attach that alone on the head but you could
screw tightly in (the bottom of that) another tiny adapter (1/4
male to 3/8 female), Manfrotto (?) number 148 on the sub-page
of the 3367/088 page.

Or would that be too wide?
