I am a relatively new user of an EOS IV. A few days ago, I exposed a roll of
Kodak Elitechrome 100 using a variety of lenses and a variety of modes -
manual (with flash), and AV (with and without fill - in flash).

On return from the processor, the whole film was found to be grossly
underexposed. I sent it back to the processor for analysis and he said that
it appears to have been properly developed but around 3 stops underexposed.

It's possible the auto DX reading did not function correctly due some body oil or 
something on the film cannister, this has happened to me twice over the last 2.5 years 
with my EOS-3, first time I caught it because I was using ambient lighting and the 
aperature/shutter speed combination just didn't make any sense, so I set the 
aperature/shutter speed manually and got great result.

It wasn't until 1.5 years later when I encountered it the second time, and since I was 
using E-TTL flash with manully set aperature/shtter speed, I could not tell until I 
got the prints back the flash was only outputting enough power for what it thinks an 
ASA 2000 film when I only had ASA 200 film in the camera.

I was able to verify this by checking the ISO setting on the camera before I put in 
another roll of film.

The moral is always double check the ISO setting displayed on the LCD as the film is 
being loaded, you just might catch a problem in time !!
