Isn't it something to do with film and CCD/CMOS chips having differing
reflectivity?

Exactly. The new Nikon D1X and D1H have to use the SB-28DX, a flash
specifically designed to work with them.
The Kodak DCS760, although based on the Nikon F5, has no Flash TTL (because
of reflectivity problems, even though you would think since the F5 used a
CCD as the metering cell it would be simple to implement some arrangement)
and uses the SB-28 (non-DX) which is an auto flash where the flash meters
the light from its own sensor.
Metz, Quantum, and others are building a hot shoe module that will transfer
the aperture setting from the camea to the flash but the flash will still
use its own sensor.
