Does your target have regularly spaced lines or some other kind of fixed pattern? It's possible for the "rangefinder" part of the AF system to lock onto different repeats of a pattern, and have major focus errors as a result.

With a brick wall this can be possible also - as you have used a camera with a coincident image rangefinder, you can imagine how it's possible to align the next brick over with the main image in the rangefinder spot.

        Just a thought...
