Cleanup/fix disconnect handling.
Bit of overhaul in regards to handling disconnect events, especially
attempting to avoid loss of any data thought to have been successfully
sent.
Also introduce (compile time) option to block writes from returning
until the work completion associated with the RDMA write work request
has been received. Performance takes a pretty big hit, but this allows
the application to "fail fast" instead of thinking it wrote
successfully. Default is currently to let the application proceed, only
detecting failure on a subsequent write (i.e. high performance option).
