It would be nice if you did manage to find out what's going wrong. My first reaction was "Oh, no, not again!" as it seemes that this kind of problem resurfaces again and again... One thing I know must be done is handling these erroneous cases better. Right now, there's no graceful degradation. This would possibly help pinpoint the problems more easily.
