No - it is not a common error :).
Probably related to the datum/projection and the gcp's that are defined.
Our support office in San Diego should be able to help you out
(you might need to send them the input header file).

whenever i select the option to use the optimum extents as the geocode
output, it calculates the top left and bottom right extents to be the same,
thus making the output size 0.  all fiducials are defined, and the RMS error
