I think the third version is the most readable. The addition (w.r.t. the
second version) of subsection headings is useful: if I scroll for more
than a couple pages without seeing a section number, I can lose track of
where I am.
In the fourth version, in the Semantics section, each production is
basically the header of a mini-section, so I think it would be an
improvement if the productions stood out more than they do. E.g.,
put a horizontal rule before each, or indent the subordinate text.
