The Mysql docs should allow you to figure that part out. Another thing to check is if you set up the db for the correct user. With Postgresql you need to create the database and grant permission to the httpd user before you can configure phpgroupware, I asume it's the same with Mysql. Also check and make sure that phpgroupware is looking in the right place for the mysql server.
