I'm not sure I have the working not working combos right.  And it's not
clear when things stopped working.  But,

Working:

1) Linux w/ LJ4 driver -> LPT? -> LJ5L
2) NT w/ LJ? driver -> NetWare Print Server -> LPT? | JetDirect -> LJ5L

Not Working:

3) Anything SMB -> Linux Samba -> LPT? -> LJ5L
4) W95 w/ LJ? driver -> LPT? -> LJ5L
5) W95 w/ LJ? driver -> NetWare Print Server -> JetDirect -> LJ5L

Assuming you aren't using CUPS with Samba, for case:

3) The client OS needs to have a native driver for the target printer
and use a raw printer on the Linux Samba Server.  The name for the raw
printer must be distinct from the printer name you normally use when
printing from Linux.  The raw printer filter should pass data directly
to the printer.

4 & 5) Look like a bad LJ5L driver in W95
