Communiy Village Eligibility Form from each and every group or individual
in the Doors of Expression needing passes (wrist or day) for the Fair,
because we're your Booth Co-Coordinators and have to turn them in at 7 PM
at the Westmoreland School at the Community Village meeting.
Those of you who need to fill one out but haven't yet picked one
