On Debian:
% dpkg -S /usr/share/man/man8/mkswap.8.gz
util-linux: /usr/share/man/man8/mkswap.8.gz
So the package is util-linux. In the documentation for that package,
in /usr/share/doc/util-linux/changelog.gz, I found this.
