I used htmldoc www.easysw.com/htmldoc

It turns headers into bookmarks and hyperlinked table of contents. It uses the fast and light toolkit. If you pay 100 usd you can get a command line version as well.

I converted the text utils html manual to pdf. It has a table of contents, links, and bookmarks. I can put it online if anyone is interested.

Did you do this?

	apt-get source textutils
	cd coreutils-4.5.4
	tar xvfj coreutils-4.5.4.tar.bz2
	cd coreutils-4.5.4
	./configure
	cd doc
	make pdf
