I have lived in India for a couple years and have seen first-hand how
safety is flouted routinely. For example an extension cord simply
consists of two single wires that each get twisted around the prongs of
the appliance that need the extension, then run across the floor
(sometimes occupied by a busy celebration crowd for a party) and each of
the two wire ends get fiddled into the holes of the nearest outlet until
power is established...

You can find distribution transformers on concrete pad sitting next to
the street and on some both high- and low-voltage feeds can be touched
by anyone walking by. People and animals learn not to do that...

Sometimes the low voltage (3x 230V) distribution network is built first
and then the buildings along the street. Some windows allow using the
wires as clothes lines...

Many more interesting situations, this is just an example.
