Most likely an assembly error which slipped through inspections. Loose connections or broken bolts. Anything can and does happen.
One can expect Tesla to identify and fix the problem. They've done so every time as far I know. Tesla could put a gap between the car and the battery so one can cool the pack with water. Could be accessed from the frunk easily. Why burn down the whole car when it can be super easily prevented?
