Marco,
A somewhat common failure that I encountered was with adapters that were
supposedly official ones, but where sideways strain on the adapter put
enough force on the ultrasonic weld that it split open, leaving either the
bottom with exposed pins in the vehicle inlet, OR the pins could remain
stuck in the charging pistol, so the charger power pins were now exposed to
the world.
The design of the adapter had two plastic pieces, that captured all pins
between them, but a break would leave the complete pins in either shell,
when you pressed and glued the two halves together you'd have a working
adapter again.
This was with the ubiquitous "Tesla" adapter that allows a Tesla to charge
from J1772 stations, but I am sure that the problem is not unique to that
version.
