This seems another case of a form using JavaScript and XFA-Forms.

It is a known bug in Poppler, the PDF library used by Evince (it does
not support JavaScript nor XFA):
https://bugs.freedesktop.org/show_bug.cgi?id=14265

More details in comment #9.  Although in the document you attached there
is more JavaScript code involved.  You can check it by yourself by
running:

$ qpdf --stream-data=uncompress 1113-Form1LLP.PDF output.pdf

and then opening output.pdf with any text editor.  Search for Javascript
or XFA and you will see what I mean.
