I found what the problem was. There is a "subscribed" file in ~/.maildir/ names courierimapsubscribed with a list of "subscribed" folders. I never subscribed or unsubscribed to any folder. It appears that courier adds any new folders created over IMAP to this file. However, for any folder that was created via procmail, it was never put in this file and would not display. The funny thing though it that some IMAP clients like sylpheed and mutt would display *all* of my folders, while clients like Evolution and SquirrelMail would not.
