I have two POP accounts and the same happens on the one that's not
flushed every time, whenever another process modifies the mailbox on the
POP server. For example, if I open the mailbox using mutt in a shell,
Evolution will surely try to download all messages again.

Are you ever using any other mail reader or tool on the mailbox?

Rather than a bug, this is a shortcoming of the POP way, IMHO. If you
really want to keep your messages on the server, you should consider
IMAP.
