Make sure you have enabled "Check incoming mail for junk" under "Edit | Preferences | Mail Preferences | Junk | General". If you use IMAP, also make sure you have enabled "Check new messages for Junk contents" under "Edit | Preferences | Mail Accounts | Edit | Receiving options | Options".
