Your client (or your client's recipient) has a Cisco PIX with its MailGuard feature enabled. Basically rejects any attempt at talking ESMTP.
Either force communication with this recipient to use plain SMTP, or convince them to fix their config :) A couple of references: http://archives.neohapsis.com/archives/postfix/2000-02/1045.html http://support.microsoft.com/default.aspx?scid=kb;en-us;q320027 You can find many more by googling for ESMTP pix mailguard fixup.
