Situation means that 2003 will have to be child. It would just have
worked so well if there was a registry hack to tweak the ADC. Never
mind.

Better yet, accelerate your migration to one organization! Absolutely.

Configure this so that you have a parent-child arrangement.  In the
child organization, give everyone an additional SMTP address of
child.domain.com.
Use that child address as the contact address in the parent.  Get a
product such as:

1.  HP LDAP Directory Synchronization Utility (LDSU)

2.  Microsoft Identity and Integration Server

3.  MIIS GAL Sync tool (limited but free)

4.  SimpleSync

5.  MS Mail Dirsync (unsupported by Microsoft, but is supposed to work) 

6.  Active Directory Connector (in interorganizational mode)

7.  Your own code

to synchronize the directories.

I have inherited seperate 5.5 and 2003 Exchange orgs sharing the same
SMTP domain name. SMTP routes into the 5.5 org which forwards it on to
the 2003 org as the 2003 custom recipients are defined by their X.400
address.

At the moment directories are synchronised manually which is rather
painful.
I would like to use the ADC but that creates the custom recipients in
5.5 as SMTP recipients but with the same domain suffix that the 5.5 org
thinks that it is responsible for, so it gets very confused trying to
deliver messages to the 2003 users.

If the ADC can be configured to create contacts and custom recipients as
X.400 users then that would solve my problem. Anyone know if this is
possible?
