This is silly, but it took looking through about 20 articles to finally find it. In IIS 5, when you have SP 4 for Windows 2000 installed, the .htr documents need to be handled by asp.dll. In my case they were being handled by 404.dll! Why, I'm not sure, but once I changed that it now works. Granted the link to close the window afterward doesn't, but that's another issue...
