You need to talk to the firewall admins to ensure that the only box allowed to receive smtp connections from the outside world is your email gateway box.
Failing that, you could use IPFront (http://www.hernanracciatti.com.ar/papers.html) to generate some IPSec rules for your Exchange box to stop outsiders from connecting to port 25.
