We are using Borderware Mxtreme appliances which utilize Brightmail (as an option).

I cannot say enough good things about this. I use it to clean 10 different domains worth of mail and it is one of the simplest hardware solutions I have seen. Previously I used a combination of Symantec on the Exchange server with a bazillion rules that I was constantly tweaking, and a myriad of client side stuff.
