Exchange 03 on Svr 03, but this problem is regarding Terminal Services on another 03 server as well.
No problem logging into TS, but sometimes, and of course sometimes means the president of the Museum, when he clicks Outlook it won't let him in. It asks for username and password but that doesn't work. This has happened to others, not only him, but not me, and I use it several times a day when I am not here. Sometimes rebuilding the profile does the trick. Re-booting the server also does the trick.
