I've found it to not be a permissions problem at all, but a limit on how many objects MAPI can recover. This article helps explain the limits, but not your problem.
To work around this MAPI limit, just use OWA to recover your folders or messages. This works 100% of the time when Outlook won't recover the folders on my servers.
