I believe I finally have this problem solved. Once I upgraded to
Exchange 2003 SP2 I could install and use the Exchange ActiveSync
Administration tool. With this tool I was able to search for devices
that were producing errors in the event log. The problem devices had
either 2 or 3 partnerships with the exchange server - one of the
partnerships was up to date, the other's last successful update was
several months ago. The devices working well only had one partnership. I
have deleted the old partnerships and I will see how the event log goes
tonight, but I am pretty certain things will now be cleared up. 

The SP2 patch went without a hitch on two servers also.

day long. I have even done a hard reset on one of them and get the same
results. In the setup for active sync on the device the "device address"
is clearly set correctly, not '22222' as indicated below. Any idea on
