We have used FreeBSD/Postfix/Amavisd/SpamAssassin/Clamav on a pair of
servers for years with no problems.  The spam filtering and initial AV
scanning is a plus and it even supports Bayesian filtering if you want
to enable it.

But, that said, if you aren't comfortable with a *nix variant you won't
like FreeBSD any better.  Once it is configured their almost impossible
to break, but getting the initial config just right can be problematic.
Then when you sit on it for over a year and decide its time to upgrade
the SpamAssassin or some other component it can be a b*#ch to remember
what you did the first time ;0

If you don't want to learn an interface like this an appliance is
probably the way to go.

Sourceforge has a spam/gateway that sits on Windows called "Fluffy"
named after the dog in Harry Potter.  It is fairly easy to configure
sits on IIS/SMTP so it is familiar to most Exchange/IIS people, but I
can't speak to how stable the software is.  The last revision I looked
at didn't run as a service and had to run active in the console, but I
haven't looked back at it in over a year so that might not be so
anymore.

I guess a lot depends on any specific requirements, but IMHO you could
do much worse than some form of *nix/bsd running Postfix.
