It could also be that he has a Delegate or Admin that has access to his
account that could have accepted for him.

I have a user that is having meetings pop up on his calendar that he did
not receive an email request for.  Is there some Auto-Accept feature on
Exchange 2003 or Outlook 2003 that he could have turned on???  Just
taking wild guesses here.
