I don't know why it happens, but I know what I do when it happens, which might not be possible in your environment. I move the user account from one domain to another and then back. That seems to kick it in the butt and remind it to properly create the Exchange mailbox with the appropriate permissions. It only happens occasionally, so I haven't expended a lot of time troubleshooting the why.
