I use Exim-SA-Clamav- site-wide config and the sender IS on the global whitelist

Perhaps one has the address in their whitelist and the other doesn't (if using personal whitelists/blacklists) on an external Spam gateway or service? Just did a telnet to your SMTP server address and noticed its using Exim as the MTA.
