I use it.  Used it starting it was called I Hate Spam Server Edition.
First installed late 2003/early 2004 in one environment.  I really like
it.  Does an excellent job and very flexible.  The AV seems to do a
pretty good job and it's included.  Define different policies, assign
users to those policies.  End user black and whitelists.  

It had some problems back in the day with keeping up with HEAVY SPAM
loads.  The processing queue would get backed and since it relies on an
SMTP Event Sink, it just wouldn't be able to keep up.  

The biggest downside is that it has to run on every mail server where
mailbox stores exist.  This means you need to allow spam to get to that
server before it can process it.  That's part of what caused the
problems I mentioned above.  I can't really speak about real world
experience with heavy spam loads under Ninja because I ended up putting
a Barracuda in place. I still used Ninja for my users who got hit the
hardest would spam, as Ninja would grab things the Barracuda didn't.  I

Having some kind of spam prevention at the SMTP Gateway/Exchange
Front-End/Perimeter/Edge to combine with Ninja would be ideal if you
wanted to use Ninja as your primary spam defense.  I'd recommend an RBL
and/or a Directory Lookup.  Both which can be done free with Exchange.
In my personal spam experience, using a trusted RBL and a Directory
Lookup check cut down on over 95% of the spam. That's a lot less spam to
actually be processed by Ninja's analytics.
