Thank you Adam for this tip.
This is the first time we have ever seen this and I was stumped.

Finally met with user, and found indeed a lot was cached in credential manager 
GENERIC regarding outlook and removed it all from the vault and had user reboot.
No idea how this got into the vault, user does not know, but moving forward.
This was clearly the primary issue ( as I am sure it was storing the OLD 
password) and a couple of days without the continuous prompts.

Check the users cached credentials within the control panel / credential 
manager. Clear this out, and then try again.

a long time with no major issues)
Cached Mode
OUTLOOK CLIENT - Security Logon Network Security = Negotiate Authentication
Seconds Until Server Connection Timeout  = 30

User reports this problem happened after required password change.
User does shut down computer each night.
User reports that after entering credentials (the new password works) in the 
pop up screen, this works for the rest of the day, but the cycle starts again 
the next day.

I am wondering if there is some "cache" on user box somewhere that is causing 
this?

Hoping for some input to get me on the right track, as no other users have 
reported this issue to date.
