What I was looking for is bouncing back any email to sender telling them that 
this mail is not read and have emails that have reference numbers in them 
continue to the mailbox. I know I can bounce back an ugly status code but was 
looking for something a little neater.

'prepend message subject with string'. An include the optoin 'except when when

It looks like you can create a standard transport rule:

Apply rule to messages:
Sent to <specific people>
Send rejection message to sender with enhanced status code
Except when the Subject field contains specific words

Is this not sufficient?
