We use Mail Essentail for Exchange 5.5.  I haven't had any problems with
it, the a/v goes out and ftp's the latest viurs definitions at some off
hour and sends me the notice.  It can block atachments and will also scan
attachments for macros and clean/quarantine them.  You can also set it up
for anti-spam.
