I don't have a good answer. From your description, I would suspect a network issue.

I've gone as far as uninstalling our AV software on the server and running a test and the problem still persists. (I stopped the IMS and no users were on the system). From what I've read on the RPC side that's only a problem if the messages aren't leaving the outbox at all. Is this true? It doesn't matter if the recipient is part of the GAL or the message is to be routed through the IMS, they all hang. I've also noticed that my server is hanging at the Windows NT screen upon reboot.
