Hmmm, I never used Exchange 5.5 OWA on a Windows 2000 /Workstation/. But I believe this should work.
Are you saying OWA on this Windows 2000 WS works? In this case I'd focus on the IIS virtual directory settings. You did create a new IIS virtual directory for the phone list app, don't you? And make sure this IIS virtual directory is not a subdirectory of another IIS virtual directory.
