I just got off the phone with Trend about upgrading and they recommended going to the AVAPI version since they were experiencing problems with the ESE. I was hoping to upgrade to the ESE later this week. I'll have to call them back and inquire about what issue they have with their ESE version.

You get access to not only the attachments, but also the message body and sender/recipient information using the ESE method. I don't believe that eManager works with 3.72 yet (could be wrong, feel free to correct me) so the content filtering aspect might not be attractive at this point, but knowing who the sender and recipient are of infected messages can be a useful feature.
