We use MailSweeper for SMTP.  Reduces (but doesn't eliminate) spam and can
be configured to block pornography (however you define it) and attachments.

Be forewarned that content filtering is a process - you're going to have to
check and clear out the filters on a regular basis.  We do it at least once,
usually twice, a day.
