For some reason your form is becoming a one-off form. You either 1. didn't publish the form to a public folder or organization forms library 2. are doing some creative coding on the form that causes it to become a one-off form 3. are changing properties of the form while you're sending it that causes it to become a one-off form (like setting a delivery exception or requesting delivery receipts)
