It seems that folks at http://www.intermedia.net/ have figured it out. They bill based on the total disk space used by a customer. Although their setup seems to be based on Exchange 5.5 - it smells like 5.5 and link to OWA brings up the yellow welcome page.
Here is what they say: Intermedia.NET tracks the disk space utilized by all mailboxes and public folders on your Exchange account. As a courtesy to you, we will automatically send warning messages to the contact email address on file when utilization exceeds the amount of Total Disk Space Available listed below. 100 MB's of storage is added to the Disk Space Allocated total below upon creation of a mailbox. To check the amount of disk space utilized by all mailboxes and public folders please click the "Disk Space Utilization" button located at the bottom of this page. Additional disk space is $5.00 per block of 25 MB. To purchase disk space please select the amount of additional disk space you wish to allocate to your account in the drop-down box below and click the "Save Changes" button. Anyone from Intermedia here?
