The likely issue is a tape drive subsystem misconfiguration, likely a driver
issue.  Make sure your drivers are up to date and use Windows' drivers
instead of letting Backup Exec take over the devices if at all possible
(Veritas techs suggest this).  

How do you know it will "eventually do it again"?
