PerlMX ??? ok for your desktop and small setups I guess, but if you are serious about performance etc. then you need to use Sendmail's in-built (if you build it to use this feature that is) MILTER (if you think of smtp event sinks in Exchange 2000 then its _sort_ of the same thing) and then the MILTER filter has to be written in c . anything else is a poor second. Of course this is all free, but you have to know what you are doing ..and it isnt related to Exchange at all so its off topic!
