When I was a messaging consultant I worked for a very large oil company (well over 100k seats) that had scanning done on the SMTP gateways and on the desktop, but nothing on the Exchange side (something like 120 e2k servers). From what I remember they had not had a email born virus outbreak\infection in years. But the way they got away with this was being very strict on what machines were allowed to plug into their network. Normally I recommend 3 levels, but I have to say this oil company seemed to make 2 levels work (each using a different AV engine).
