They block virus and/or attachments at the IMS level before they hit any mailbox/machine/server.  They also can be set up to send the offending party a email message stating that they are sending out a virus. This last feature will cut down the number of virus you get significantly. Many organizations upgraded their AV after the sales/marketing/CEO type get a message back stating that they are spreading "X" virus

I'm using Symantec Exchange AntiVirus 2.1.  It allows you to block attachments with a registry hack, it doesn't seem to let me blcok more than 10.  I guess that should suffice, I was just wondering if there was something out there with more advanced features.  It will probably cost $$$ though....

If you have the lastest security patches installed, you can use the FREE admin pack to specify what file attachments to block.
