Do you have your exchange server set to retain deleted items for a certain amount of days so they can be undeleted? And did your users empty the deleted items and sent items folders? Sent items is a huge chunk of space if it's not cleaned out!
I don't believe I've ever seen a huge amount of space reported after online defrag, but after a good clean up and running eseutil I can often recover 600 or 700 mb of free space. I vote to run the eseutil anyway.. I'll go down 3 flights of stairs and check to see how much white space my server is reporting today..because I know that there's hundreds of megs of space if I run eseutil right now.
